Juggernaut build advice

My Build follows Phoenix's Molten strike juggernaut

Up until maps everything was great but made a few changes(which I subsequently reversed) and everything turned to custard :(

I'm bleeding or losing health very quickly it just drains out and I can't see where in the build I have gone wrong??

If anyone can see where I screwed up, appreciate a heads up.
