Please check my build - Boneshatter Juggernaut

Please can somebody check my build and tell me how to improve it or fix it, if I have something bad.I just really dont know.

I tried to make it right according poe ninja and get good gear.

1)At POB show damage 479M at poe.ninja 280m - why such a big difference?

2)Still dying - endgame content - bosses

3)Cannot do for example Cortex map, better quality any of breachstones

4)Character is slow
