Your damage is very low.

Bleed is not in a great place right now and needs significant investment to make work. I would suggest swapping from bleed into phys->cold conversion and cold->fire conversion using hrimsorrow gloves and heatshiver helmet. With a little crit chance you will find your damage is much higher.

Also consider using the new earthquake of amplification gem and scaling up some duration on it.

Also consider changing your sixlink leapslam for cyclone with shockwave and using frostblink with a hextouch curse and bonechill for movement. Cyclone procs shockwave nicely and gives you phasing without needing phase run.
