LF help by my berserker warcry ground slam

Hello. I recorded a video where I run through a sample map. I'm wondering what I'm doing wrong that my character dies so quickly. The action starts around the 3-minute mark in the video. The problem also occurs when using "leap slam." You can see how much HP I lose during jumps. Could someone with more experience comment on this and offer some advice?

I'm also attaching my PoB, although something isn't quite working because I get three warnings related to helmet buffs, even though I can actually activate them all at once...

Actually, no. My Pride doesn't work because I'm short on mana... Should I give up something else?
