Legacy of the Vaal Bugs

Hi,

I cannot progress on this quest, it asks to place small soul core into stone altar, but the door is already been open and no button avialable to insert the gem and open it again. The map cannot be refresh it again,even exit the game.

And the quest cannot be skipped, pls help!!!!
