monsters remove 3% of life/es/mana on hit

this league first time I play CI build.

I run this mod and somehow sometime monster seem instantly kill me while I still have energy shield.

But my friend he ran a lot of risk scarab with int stacking KB build. he said this mod cannot bypass energy shield. he never die because of that.

so a question is, is this mod can instantly kill you if you play CI build ?
or may be monster gonna kill you every 34th hit (stack multiple 3% and kill you if it reach 100%)

isnt 3% of 1hp is 0.003 hp?
