Skills being removed from hotkey every instance change.

So, first time this happens to me, I made a new character, a shaman, when I change instance (enter a map, exit a map, or whatever it is) the skills are removed from the hotkey binding, and every instance I need to set the skills on each key over and over again to the point that I am 2 days into this character and I am going to delete it.

I tried it all, even deleting the game and reinstalling from scratch. nothing works, new gems, locking weapon, weapon sets, nothing seems to work.

Did someone else got this issue and if so how is the fix for this problem?
