Stuck in void after using portal, or dying, to return to hideout.

When returning to the hideout from a map via portal or death, the map device and my character are placed far out of bounds from the hideout in a void. Unable to move, have to relog to reset character into hideout. Map device is still outside of hideout and needs to be manually replaced into hideout.
