Maybe you got bad loot and filter didn't show bad items.
Really unlucky but I sometimes get so bad loot that boss drops 0 loot that goes through filter. You can try to press "ALT" sometime to see maybe you had items but they were bad. |

Just had this happen to me on a T3 map. Crimson Shores map with the Earth Stomper boss from first floor of Sekhema. Zero drops.
