Unable to Return to Hideout, Lost all 5 remaining shaper fight portals

While fighting Shaper on Expedition Softcore I died on my first attempt. The game told me unable to return to hideout when I clicked the return to checkout button. When I used Return to Town then went back to my Hideout my 5 remaining portals on my map device for "The Shaper's Realm" were gone. I also received the following message in my chat: "Error: Failed to join instance because: Disconnected from Server. Error: Failed to join any instances." Hope this helps with fixing issues!
