Is the Controlled Blaze support bugged?

My chance to hit: 94%

My chance to ignite: 90%

Skill applying the ignites:
Glacial Hammer of Shattering - Ignite Proliferation - Ruthless Support - Controlled Blaze Support

Tincture: All Damage Can Ignite


I'm a Champion that weapon-swaps to proc Adrenaline with Corrupting Fever. About half the time the ignite indicator from the Controlled Blaze support is showing me stacking ignites. The other half is showing nothing while I'm attacking and the target is obviously ignited. Is there some sort of bug with the Controlled Blaze support? Or am I understanding the functionality wrong?
