Cannot interact with portal (Poltergeist portal MTX)

Portal scrolls stopped working in map. Other people in map also cant open a usable portal. Portals which they open are all the same MTX (poltergeist) even when i unequipped it or changed to other MTX. Only notable event in map was death in Ultimatum. Later in map I opened syndicate ho and the portals to it and out of it worked. After getting to map from syndicate the portals started working again.

Happened again.

I've opened a portal in syndicate lab and it worked then died in map and after coming back to map the portals stopped working.
