Loot Gone!????!?!?

Got put back to character select mid-map.
Joined back.
Map closed........but also my inventory cleared of the loot i picked up.

Started new map.
Finished map.
Portal back.
Loot gone from inventory again......

What in the name of all that is holy is going on??
