Omen of Amelioration not consumed on death

Hey,

I died in a map with an Omen of Amelioration in my inventory and it was not consumed, and I lost my experience. I had bought another one (having died about 5 minutes earlier and having that one consumed) but this time it did not work. I died in a T16 Basilica map to a delirium mob, if it matters.

You had already consumed an Omen in that map, so you cannot consume another Omen in that map.
