Omen of Amelioration Not working

Started an ele reflect map without paying attention and died almost instantly. Was ~5% from a level and dropped to 15% my omen was still in my bag and was did not apply. I had not previously died in the instance so it should have been consumed.

Were you also carring around another omen?
Death door, or Return? You can only have 1 omen used per instance. | |

This actually happened to me twice in a span of last 13 hours. Didn't have any other omen in the inventory.

Can confirm the same thing has happened to me multiple times now. Really frustrating when I am specifically farming for XP and the item I buy doesn't work.
