Missing Ultimatum in Map (Did not spawn with scarab) 1342169483

Using the following scarabs in my T16 sepulture map, an ultimatum did not spawn. I explored the entire map, killed the map boss, re-explored the map, no sign of any ultimatum anywhere. Scarabs used were Ultimatum scarab x1, ultimatum of bribing (2), and ultimatum of inscribing x1. Other mechanics that were present were harvest and ritual. No master missions present.

Happened again. Confirmed that ult should spawn 100% due to passive tree. Ultimatum did not spawn in map.

Another one... this is really frustrating. I don't know whats causing it, but its random. Just like my last one, another case of "100% chance of ultimatum" and it doesnt show in map mods, or show up in the map at all. Hope this gets fixed. Wasting scarabs and maps and potential loot is discouraging.
