inscribed ultimatum did not drop

the ultimatum from xyclucian did not drop have done the boss 2 times and still not get it quest tells me to go place it in the temple offering but there is no item in my inventory or stash that will go into the slot

i found a work around if you party with someone who is runing it then you can accend and the quest goes away
