quest item disappeared

I just dropped the left part of a relic (quest item) at the trial of the sekhema on the ground but it completely disappeared (even without a lootfilter). Not on the ground nor in stash or backpack

All other items seemed to be fine (drop normally on the ground), only this green quest item

Same here, after boss kill, quest item disappeared at some point, while being in the caravan, the second part of the puzzle (left relic), can't find it, can't continue playing. Please help.

Can you please make the boss be dropping the quest item every time?

I cant play anymore because of this issue. Im stuck at act 3... Dropped large soul core.
