Augury atlas map - Not completable

This is the map where you need to pull 3-4 levers to open the center of the map. In this instance there was 3. I opened all 3 levers that were on the map and the center remained closed.

Circled the map 3-4 times, and there is no visible 4th lever. The levers are usually at hallway entrances so there shouldn't be a 4th. Triple checked the 3 are pulled.
