Can't complete Campaign

I finished the campaign on my second character but when i try to enter maps, the game tells me i still have quests to complete. When i looked over the acts, multiple quests that i'v already completed, appeared as incompleted. For example:

In act 3 normal, i had a quest that makes you travel to cruel difficulty that i obviously completed since i finished the campaign. When i did it, it reset the world map of act 3 normal.

In act 1 normal, i have a quest "The mad wolf of ogham". I have to speak to the Hooded one and travel to act 2, which again, i already did. This time tho, the npc doesn't have a dialog option that would complete the quest.

At this point my whole campaign is a mess. Map in some acts is reset, some quests just reappear out of nowhere. I completed the whole thing but can't access maps, not sure what to do.

I'm also having this issue.

On my second character, I have "The Mad Wolf of Ogham" uncompleted and I cannot enter maps. I also noticed that on my first character I have "Legacy of the Vaal" to travel to Cruel difficulty uncompleted and I'm able to enter maps just fine.

EDIT: Found a fix. I had to go back to The Ziggurat Refuge and open a map in the map device there before opening maps in my own map device. There doesn't appear to be a quest indicator for this on the map or elsewhere.

Have completed normal and cruel difficulties. Quest "The Mad Wolf of Ogham" stating "Travel East" in Act 1 Normal, and showing as incomplete.

Unable to open maps, "You cannot access endgame content until you have completed the campaign."

edit: saw edit of second post, also resolved my issue

+1 second post worked. I was unable to teleport directly to Refuge, had to go to act 1 then teleporting to the refuge worked!

I just had the same just now (2nd character etc..)
However the block only triggered - along with the message - when activating it from the hideout map device. The map portals opened from The Ziggurat Refuge no problem.

Had this same issue as well on an additional character after unlocking a hideout. If you go to Ziggurat Refuge it will work. Only have to do it for the first map.
