Inscribed ultimatum : 10 trials - Cannot place the last deadly fate

I have run Inscribed ultimatum 10 trials three times. first and second time i put the fate in the mask icon. then when i wanted to put the third piece in the mask the third time, the mask didnt show up. i have the right piece and the middle piece in the mask, and i dropped the left piece on my third attempt but the mask doesnt appear for me to put it. the door remains closed and i cant interact with it or anything. i was standing right in front, i moved aroud a bit and even tried dropping the fate on the ground but i couldnt do nothing.

i took the corrupted items from the end of the trial before trying, maybe that count as forfeiting or w/e but im pretty sure i also took the items on my first two runs where i could put he fate in the mask.
