No charm drop after Tribal Vengeance

I killed the witch and got the skull, but after talking to Servi, it said "take your reward" but I didn't get a charm. Act 3.

I'm having the same problem. It says to "take the reward from Servi" but there is no reward and I can't complete the quest.

I know it has been a few months since this was posted, but I came across this bug just recently.

Servi is standing in The Azak Bog zone near the waypoint, which would lead you to believe that this is where you would get your quest reward, but the marker for the where the quest reward is located is actually placed on the Ziggurat Encampment in the waypoint map.

If you go back to the Zuggurat Encampment and find Servi, you can get the quest reward there.
