Random weapon swap

Hello,

I'm sorry about my disastrous English.

I'm here to tell you about a bug that's only happened to me since I've been in ‘endgame’. When I arrive in a map, or one with large mob packs, the map loader randomly swaps my weapons, and I find myself without minions (infernal witch), which means that I die immediately due to the number of mobs.

I only notice this in endgame.
I should point out that I have a visual of them. Don't let them die. And that it's really a weapon swap. I've changed the key for greater safety but nothing happen.

I have a similar issue. Just started new Minion Witch and its constantly doing Weapon Swap for no reason.

I couldn't even manually Weapon Swap if I wanted do as there is nothing equipped ('Set up second weapon set in your inventory to use weapon swap' Message).

Have checked all Skills. USED WITH WEAPON SET have all been Set to I only, and on all Tabs (MINION INFO. FIRE BOMB. EXPLOSIVE DEMISE. COMMAND. etc)

Witch swaps to zero weapon, all Minions disappear.
I've been paying close attention to it, and its seem completely random.

Note, its my 3rd Character (Monk and Ranger before) and they NEVER had this problem. I've not even Lvl10 on my Witch and its happening very frequently.

* FURTHER TESTING - Weapon Swap will tricker almost every time the Boss Fight sequence triggers... which is a real kick in the nuts! lol
