the Ultimatum for the temple offering disappeared locking me out of trials of chaos

i picked the ultimatum after i defeated the boss in chimera wetlands when my inventory was full. it said that i picked it up but it was nowhere to be found. i cannot get access to the trials of chaos because of this.
