Game crash map nodes gone and all loots gone within my inventory

I Losing Despair Delirium. Please fix it i Lost a T15 note and i wasnt die

I just had this same issue in a tier 5 map. I was playing in a party with one other person and we started a breach. After about 30 seconds a large rare mob spawned along with a large group of mobs. The game froze and we could still interact with our menus and mouse over items on the ground and mobs, but we could not move and the mobs were frozen as well. We waited for around 3 minutes with no change and then closed the game and logged back in. After loading back in, the map shows "attempted" suggesting we died in the map. We also lost all inventory items that were picked up during the map. Items that were already in our inventory were not affected.

Same thing just happened to me in a T15 breach. I completed the map and opened the breach after. It dropped a divine orb which i picked up and 15-20 seconds later the game froze. I exited to log in screen to not only see that the divine orb and everything else i had in my inventory about 10 divines worth of swap out gear/gems was gone. Even the map which i had already completed was now not completed...
