Bug: can't unlock 4th ascendancy

My game crashed after completing my 4th trial. I tried to capture the video upon killing the final boss on Xbox and that caused my game to crash.

When I logged back in I was at the waypoint without receiving my points. I have since completed it two more times and I still do not get the ascendancy point rewards.

I was using item level 75 inscribed ultimatums that state "will grant ascendancy skill points" and I have already completed the other three trials so I am sure it has something to do with my game crashing after I completed the trial the first time but before I received the skill points.

Is there anything I can do to fix this?
