Map Layouts Bugged every few maps

This is the first time it has happened to this extend, but I just procced the atlas Passive to get 10 additional breaches in my map. I cleared about 5 of them and look at my map afterwards just to see that The last rare monster is out of bounds, there is like a second map layout next to mine that I cannot reach, and the last 5 of my breaches are on that other side I cannot get to. I have also noticed that in maps Like Oasis Ritual is bugged a lot of the time. One out of the rituals is always unclickable and cannot be ran for favour. I have attached a screenshot of my map layout (Decay). You can't see it on the screenshot, but the Breach mist (The purple stripes) were visible on the other side.
