PS5 - Breach map issues

Every time a map is opened in end game that contains a breach and the breach is started you get about 1-2 minutes of smashing mobs and then the game freezes and then logs you out. You lose the breach map, all items that dropped & the waystone.

+1 this has happened twice to me on PC as well. Game freezes in a weird way. You're able to open inventory, map, passives, etc. but you and all the mobs are stuck in place around you, breach wave stops moving outwards, but pulses. Eventually it will log you out or if you log out/alt+F4 you come back the node says "Map attempted" you lose all loot you've collected in your inventory, the map, etc.

If you port back and dump gear in the stash you keep that gear, but anything in inventory goes poof.
