Inscribed Ultimatum disappeared?

Hey idk where my inscribed Ultimatum is in my Inventory, seems to have disappeared. I've farmed the boss I got it from multiple times and it hasn't dropped again.

any help is greatly appreciated. I'm 12 levels beyond what I need to be for the act 3 ascendancy.
