Game force disconnects when opening a map or chaos trial

I'm on xbox and when I open a map or enter a chaos trial it eats the waystone/ultimatum. I've lost 5 waystones/modified maps and 4 ultimatums over the last 3 days. And it fails the map/trial everytime. You don't even get to attempt to enter it.
