Stone Citadel didn't drop crisis fragment

As title says. Have spent since the game launched finding these, this is my first one, no fragment.

And before I get asked, no loot filter, hit alt, etc etc etc. It did not drop.

Didn't get one on iron citadel after killing the boss

I also have not been getting fragments from any of my citadels
