Missing Quest Ultimatum

After killing the Chimera boss which drops your quest ultimatum for temple of chaos, the ultimatum dropped. My inventory was full and before i could drop an item to pick up the ultimatum, it disappeared. I have tried to kill the boss since then and the ultimatum does not drop again meaning I cannot run the trial of chaos until another ultimatum drops somewhere.
