Unexpected disconnect error when using pain offering, maps bricked

I get disconnected and receive the "Unexpected disconnection has occurred" error sometimes when using pain offering in a map. Not every time I cast it, but it's happened 6 times now and each time I had just cast pain offering as the game freezes and disconnects. When logging back in my map is gone, the node stating "map attempted".

I've had very little crash and disconnect issues the whole EA and this started happening last night.
