map bug & disconnect lost all of item . and map reset again. ( before allready complete )

hello . i just going for t16 and completed the map. and im keep going for picking item. and then the map is freeze then i choose to esc and back to main character. when im going back online. the map is reseted ( not completed yet) and all the item is gone from my inventory.
