An unexpected Disconnection occured

"Disconnected - An unexpected disconnection occured"

Happens when you use the Portal or warp to Players. Definitaly server related because it is ok when playing early morning or late, but prime time or on weekends I have a 20-50% chance to disconnect when using a portal or wharping to a Player.

Also ist does not (at least almost never) happen when loading in to maps with the map device.

Also only on Playstation, Not on Xbox.

Yeah, it happens all the time and should be number one priority. If you search for disconnect on the console bug report forum you get 37 PAGES (!!!) of results. Most of which refer to this bug but it isn't even listed under known issues.
