Omen of Amelioration not triggering

Not an issue with multiple omens in my inventory, or triggering this or another omen in the same instance. I thought maybe this specific omen was bugged so I traded it away. Had died in a while but then I did die at a high level and decided to buy another Omen of Amelioration. Died again days later with the new one in my inventory (no other omens in inventory or consumed in ages), omen didn't trigger, lost full exp, and omen is still in my inventory

Sorry if I am asking the obvious and not to deny a problem, but was it activated? (right-click omen when in inventory)

Also was it in your main inventory, if you happen to be playing the Titan ascendancy with the extra 20-slot chest?

Several users have been reporting issues with these omens being unreliable.

Oh my gosh - I didn't even realize the omen needs to be right clicked in your inv to make it active. Not how it was in PoE1 and I had no F'ing clue that was a feature. Thank you good sir. My lvl 93 Monk is gracious
