Breachstone map empty

Hello I used a breachstone, entered the map, activated the breach hand but no monster was showing up. The timer ended and I was teleported back to the hand on which I was immobilized and getting hit by invisible monster.
Also, I tried reaching the end of the map, it was showing a continuation on the mini-map but in reality it was a dead-end, and with cursor going over the supposed continuation of the map I could see invisible monster.
