Crown of Stone quest progression locked

I am missing 1 of the pieces but I have touched all 3 shrines. I have even let the zone reset and cleared again but Im still missing a piece to continue. On my Witch toon. I did die after I picked up the second one but when I went back it did not drop again and was gone out of inventory if that helps,
