Monolith Arbiter Disconnect

After beating Phase 1 screen froze and our party of 5 all got disconnected. Upon re-logging back in we have lost the fragments and unable to re-enter.

Vanyalwen managed to get a clip on xbox of us.

Unsure whether me typing /bug once we reconnected has saved any type of record of this.

Anyone else had issues with monolith ?

not precisely this but idk why, but I get disconnected from the server (and it suddenly started happening when it wasn't before), in maps when going up to boss mobs a bunch and it makes me lose the maps so I cant get the boss loot a lot of the time. Some disconnect protection would be nice I guess. its also like endgame is more unstable than campaign for some reason.
