Overpowered Poison DOT: Illustration of a mistake in no EA damage log

I just encountered a poison-based source of chaos DoT that killed my character in less than half a second. With an EHP pool of a bit over 3.7k (no ES), and Chaos resistance of 24%, that's about 10,000 damage per second.

I estimate the time at a maximum of .5 seconds because I had barely enough time to see my life pool turn green, and everything drained so fast that I was barely able to recognize that it was DoT and not a hit. It was very likely less than half a second. Needless to say, I didn't have reaction time to activate a flask (and that wouldn't have helped anyway).

It is possible that the damage source was a ground effect or something that triggered from the ground surface (explosion perhaps). If it was, there was no graphical indicator that I noticed beforehand.

I think the damage source was a monster of Normal rarity, but it is possible that it was Magic. My spell killed the last visible monster at about the same time, so there was nothing left alive on the screen to analyze after character death...further suggesting something on the ground.

This was on a T14 Crimson Shores map. Map mods included 25% elemental damage penetration for monsters, and no generic added damage, chaos damage, or physical damage mod.

10,000 poison DPS dealt to a player character is too much from a Normal or Magic monster on a T14 map, and probably too much from any monster in any instance.

If there were a player-viewable damage log during the EA period, I would be able to see exactly what happened and share that information in this post, instead of the above list consisting of "probably and likely". As a result of a damage log not being made available, this post is likely far less useful and it will take far more time for this OP damage source to be located and re-tuned.
