Failed map even tho i had killed all rare and boss

Hi

i just cleared the iron citadel where i killed the boss and all the rares that where showing on the map the only thing i left is one enemy. when i left the map the brown info box in the right corner said i hade only one monster left in my map and i had no rare markers on my map and i could not find the last one. But still it says i failed the map after i started a new map.

i killed the boss and got the fragment.

Edit: wanted to add that i did not die.
