Black Screen after death of one couch coop player

It's happening everytime when my wife die I cannot finish map as it's getting like on screenshot and need to die and then start new map
also notice that I can teleport to town when it's outside of fight but then we cannot comeback as she's alive in town (still didn't unlock h/o)

Very irritating as our progress is slowed down
