Campaign completed, and dead bosses are showing up as not defeated.

I just noticed that the Bog Witch is showing as undefeated. I'm pretty sure I beat her in cruel AND normal, and got the spirit reward already. I went and fought her again just to make sure, and it still shows her as undefeated on the map.
