Omen of Amelioration not working

For the second time now, I died and lost 10%xp with the omen active in my inventory. The first time was against the ritual pinnacle boss and now at the stone citadel. anyone with the same problem?

I just realized it wasn't "Active". Did you right click to make it active?

I just bugged for me, came back in hideout with it still active and it didn't prevent the xp loss.

Not sure if it's relevant, but I also had an omen of resurgence in my inventory and that triggered right before

The grey text that explains an items use at the bottom of an Omen's tooltip will say "Only one Omen can be triggered from combat in each instance" if the Omen falls under the "triggered from combat" type.

Omen of Amelioration and Omen of Resurgence both have this text. If your Omen of Resurgence triggered, your Omen of Amelioration cannot also trigger in the same instance.
