Trial of Chaos maps keep generating map layout with no pathway forward

I'll see the lever that should have the levitating platform next to it (the one that takes you to the next trial round selection). But the levitating platform doesn't spawn, looks like the main outer structure spawns in place of where the platform would spawn.

This has happened to me like 3-4 times now, many times on the last trial. Extremely frustrating.
