No access to Sekhemas

Level 64 here, multiple times killing Balbala in Act 2 Non Cruel mode, yet she does not drop the quest item needed to go to Sekhemas. I do not have access to the waypoint on my map nor the travel point on the desert map, and I am not sure how I can fix this issue. I have completed all conversations with NPCs in Caravan but still, no access.

Anything would help.
