Stuck at chaos trials platform bug

I ran off the platform accidentally as it was moving up and got stuck under it. I had to make a portal and go back to be free myself.

I found in other areas of the map I sometimes will fall off the platform into the void where there should be a barrier. For example at the zalmarath the colossus platform. At the colossus, I am a monk who places a bell. If I place the bell too close to the edge I may fall down, or the boss becomes glitched and I can't give it damage.
