Disconnect every time I try to join a map

Outside of the first quest map, every map i try to enter, fails to let me into the instance and results in an immediate disconnect.

We thought this might be a map device issue, changed it, tried again and was able to enter, the next map was unable to enter. Please let me know what next steps are needed.
