Chaos Trial - Collect all of the Soul Cores - Bugged/inaccessible layout ID#3462580318 w/ Video

During a run of a Level 80 Chaos Trial, on the fourth stage with the objective of "Collect all the Soul Cores", two of the orbs failed to spawn and the third was locked behind an inaccessible switch making the run impossible to complete. Was also able to stand in an area that I should not have been able to clip on to.
