Bug with the omen of amelioration?

I died twice, on 2 different maps, on 2 different days, I lost 10% XP each time, while I had an omen of amelioration activated in my inventory. The weird thing is that the omen of amelioration was not "triggered" but after each death it was still activated in my inventory. Has anyone else had this problem?

Do you use other Omens that are triggered by combat? (Omen of Resurgence)

As the text on the item says, only one Omen can be triggered by combat each instance.
